Digitime at KI is a wiki with lots of resources and a useful example of a Digital Citizenship agreement linked to the Key Competencies.

Andrew Churches in his Educational Origami blog has an acceptable users agreement that doesn't list the does and the don'ts but is based on the ethics of behaviour.
